After performing reconstructive operations on the arteries of the lower extremities, 40–45% of patients developed a relapse of ischemia during the next 3–5 years due to the progression of the underlying disease, 80% of them undergo high-level amputations of the extremities. Carrying out medical and social expertise in the early stages after amputation is an important link in the rehabilitation of this contingent of patients.</p> <p><bold>Purpose.</bold> assessment of medical and social expertise and quality of life of patients who have undergone amputation of the lower extremities against the background of obliterating arterial diseases.</p> <p><bold>Materials and methods.</bold> The object of the study was 87 patients who had undergone amputation of the lower extremities, who were hospitalized in the vascular department of the Republican Clinical Hospital of Nalchik from 2015 to 2019. Results: The following results were obtained during the examination of patients at the Medical and social expertise bureau in Nalchik: The third group of disability was assigned to 14 patients; The second group of disability was assigned to 68 patients; The first group of disability was assigned to 5 patients. In this case, the objective data of the state of hemodynamics in the lower extremities were assessed. When assessing the quality of life of disabled people using the SF-36 questionnaire, the following results were obtained: after amputation of the lower limb, the indicators of physical health sharply decreased, and the indicator of social role also significantly decreases, not reaching half the value of the scale (38 points).</p> <p><bold>Conclusion.</bold> It is necessary to carry out a medical and social examination of patients after amputation of the lower limbs by the visiting commission of the Main Bureau of Medical and Social Expertise in the postoperative period in the hospital, while up to a period of 6 months (before hip prosthetics) for group 1 of disability patients , considering that these patients during this period need constant outside care. It is necessary to create, on the basis of rehabilitation centers, orthopedic workshops for the manufacture of good quality limb prostheses, and train patients in the use of prostheses by specialists of rehabilitation centers. After teaching the patient to use a prosthesis in a rehabilitation center, undergo a medical and social examination to obtain an appropriate group of disabilities.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>В Кабардино-Балкарской Республике ежегодно производится до 200 реконструктивных операций на аорте и артериях нижних конечностей по поводу облитерирующих заболеваний артерий, основным из которых является атеросклероз. После выполнения реконструктивных операций на артериях нижних конечностей у 40–45% больных развивается рецидив ишемии в течение последующих 3–5 лет вследствие прогрессирования основного заболевания и у 80% из них выполняются высокие ампутации конечностей. Проведение медико-социальной экспертизы в ранние сроки после ампутации является важным звеном в реабилитации данного контингента больных.</p> <p><bold>Цель</bold> — оценка медико-социальной экспертизы и качества жизни больных, перенесших ампутацию нижних конечностей на фоне облитерирующих заболеваний артерий.</p> <p><bold>Материалы и методы. </bold>Объектом исследования стали 87 больных, перенесших ампутацию нижних конечностей, находившихся на стационарном лечении в сосудистом отделении Республиканской клинической больницы г. Нальчика с 2015 по 2019 г.</p> <p><bold>Результаты.</bold> При освидетельствовании больных в бюро медико-социальной экспертизы г. Нальчика получены следующие результаты: III группу инвалидности присвоили 14 больным, II ― 68 больным, I ― 5 больным. При этом оценивались объективные данные состояния гемодинамики в нижних конечностях. При оценке качества жизни инвалидов с использованием опросника SF-36 получены следующие результаты: после ампутации нижней конечности резко снижаются показатели физического здоровья, а также значительно снижается показатель социальной роли, не достигая половины значения шкалы (38 баллов).</p> <p><bold>Заключение.</bold> Необходимо выполнять медико-социальную экспертизу больных после ампутации нижних конечностей выездной комиссией Главного бюро медико-социальной экспертизы в послеоперационном периоде в стационаре, при этом до периода 6 мес (до протезирования бедра) присваивать I группу инвалидности, считая, что за данным больным в этот период нужен постоянный посторонний уход. Необходимо создать на базе реабилитационных центров ортопедические мастерские по изготовлению протезов конечностей хорошего качества, там же обучать больных использованию протезов, после чего в реабилитационном центре они смогут пройти медико-социальную экспертизу для получения соответствующей группы инвалидности.</p></trans-abstract><kwd-group xml:lang="en"><kwd>medical and social expertise</kwd><kwd>amputation of the lower extremities</kwd><kwd>obliteration of arteries</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>медико-социальная экспертиза</kwd><kwd>ампутация нижних конечностей</kwd><kwd>облитерация артерий</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Сергеева О.В., Филимонов А.И.
