Nonpharmacological management of the orthostatic hypotension may have a positive effect on functional status.</p> <p><bold>AIM<italic>:</italic></bold><bold><italic> </italic></bold>To create and study the effectiveness and safety new method of rehabilitation which includes wearing an abdominal binder and laser therapy for correcting orthostatic hypotension in patients with tetraplegia.</p> <p><bold>MATERIALS AND METHODS<italic>:</italic></bold><bold><italic> </italic></bold>The study involved 120 patients with tetraplegia motor level C<sub>5</sub>-C<sub>8</sub>, aged 18–50 years, time since spinal cord injury from 6 mouth till 5 years who were divided into 3 groups using randomization. Group 1 (control) received a traditional rehabilitation program based on existing Russian’s clinical guidelines for physical rehabilitation spinal cord injured patients, group 2 received the same traditional rehabilitation and additional wearing of an abdominal bandage in a sitting position during the daytime, group 3 received the same rehabilitation as group 2 additionally with low level laser therapy (LLLT) using a combined method (neck and heart points). The results were assessed at the beginning (T<sub>1</sub>) and at the end (T<sub>2</sub>) of a 30-day course inpatient rehabilitation, the drop out bias was 4 patients (3%).</p> <p><bold>RESULTS<italic>:</italic></bold><bold><italic> </italic></bold>In all groups, positive dynamics were noted in the functional, autonomic and psychological states. In 2 and 3 groups relative 1 group were noted increasing values 24-hour systolic blood pressure (SBP) (109±8.2 and 109 ±8.6 mm Hg against 106±7.6 mm Hg), daytime SBP (111±8.8 and 111±9.1 versus 108±8.4 mm Hg), a decreasing nighttime SBP dipping (10±2.1 and 11±2.4 versus 7±1.8 mm Hg), SBP loss during tilt table test (13±3.3 and 12±3.2 versus 16.0±3.5 mm Hg), increasing parasympathetic reactivity (sinus arrhythmia) (1.23±0.13 and 1.26±0.15 versus 1.21±0.11). Additionally decreasing level of depression (by 25% and 28% versus 20%) and improvement quality of life (11% and 11% versus 5%) were seen in the 2 and 3 groups.</p> <p><bold>CONCLUSION<italic>:</italic></bold><bold><italic> </italic></bold>The proposed method of orthostatic hypotension nonpharmacological management using wearing an abdominal bandage and a laser therapy (LLLT) combined method is safe, effective in improving orthostatic tolerance, quality of life, and reduce depressive disorders in tetraplegic patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold><bold> </bold>Спинномозговая травма на шейном уровне ведёт к формированию тетраплегии с грубыми вегетативными нарушениями, одним из которых является ортостатическая гипотензия. Коррекция тяжести течения синдрома ортостатической гипотензии оказывает положительное влияние на функциональное состояние пациентов.</p> <p><bold>Цель.</bold><bold> </bold>Разработать и изучить эффективность методики коррекции ортостатической гипотензии у пациентов с цервикальной тетраплегией.</p> <p><bold>Материалы</bold><bold> </bold><bold>и</bold><bold> </bold><bold>методы.</bold><bold> </bold>Обследованы 120 пациентов с тяжёлой цервикальной тетраплегией С<sub>5</sub>-С<sub>8</sub> в возрасте 28 (от 21,0 до 46,0) лет, которые были разделены на три группы по 40 человек: 1-я группа (контрольная) получала традиционную программу реабилитации, 2-я группа получала традиционную реабилитацию и дополнительное ношение абдоминального бандажа в положении сидя в течение дня, 3-я группа получала реабилитацию 2-й группы совместно с применением низкоинтенсивного инфракрасного лазерного излучения (НИЛИ) по рефлекторной и прямой методике. Оценка результатов выполнялась в начале и в конце 30-дневного курса стационарной реабилитации. Ошибка истощения составила 4 человека (3%).</p> <p><bold>Результаты.</bold><bold> </bold>Во всех группах по окончании реабилитации была отмечена положительная динамика в отношении функционального, вегетологического и психоэмоционального состояния. Во 2-й и 3-й группах относительно 1-й группы (контрольной) было отмечено более выраженное увеличение суточного систолического артериального давления (САД) (109±8,2 и 109±8,6 мм рт. ст. против 106±7,6 мм рт. ст.), дневного САД (111±8,8 и 111±9,1 против 108±8,4 мм рт. ст.), уменьшение ночного САД (10±2,1 и 11±2,4 против 7±1,8 мм рт. ст.), уменьшение падения САД в одномоментном тилт-тесте (13±3,3 и 12±3,2 против 16,0±3,5 мм рт. ст.), нарастание парасимпатической реактивности (синусовой аритмии) (1,23±0,13 и 1,26±0,15 против 1,21±0,11). Дополнительно отмечается уменьшение выраженности депрессии (на 25 и 28% против 20%) и улучшение качества жизни (на 11 и 11% против 5% соответственно) во 2-й и 3-й группах.</p> <p><bold>Заключение.</bold><bold> </bold>Предложенная методика коррекции ортостатической гипотензии, включающая ношение абдоминального бандажа в положении сидя и применение курса НИЛИ, является безопасной, эффективной, способствует повышению ортостатической толерантности, повышению качества жизни, уменьшению лёгких депрессивных нарушений у спинальных пациентов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>orthostatic hypotension</kwd><kwd>tetraplegia</kwd><kwd>spinal cord injury</kwd><kwd>abdominal binder</kwd><kwd>laser therapy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>ортостатическая гипотензия</kwd><kwd>тетраплегия</kwd><kwd>травма спинного мозга</kwd><kwd>абдоминальный бандаж</kwd><kwd>лазеротерапия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Krassioukov A, Biering-Sørensen F, Donovan W, et al.
