Abstract

Electro-acoustic correction is the basic method of rehabilitation of patients with hearing acuity disturbance as well as cochlear implantation in case of deafness. The article considered the criteria of efficiency of aerophone use, which were determined not only by the type and quality of hearing-aid itself but by the correct selection of its electro-acoustic parameters and adequate individual fitting for the particular ear including individual earphone production.

Features of disablement expertise in patients with Parkinson's disease with various age of the disease onset

Abstract

Features of determination of disablement groups in patients with Parkinson's disease with various age of the disease onset were analyzed. We examined 86 patients with PD registered at the Nizhniy Novgorod office on assistance to the patients with extrapyramidal pathology including early and late onset of the disease. Slower tempo of progressing was characteristic for the patients with early onset, and their disablement developed in later terms from the moment of the disease development than in patients with late onset. Earlier disablement of the patients with late onset of the disease depended on more rapid development of postural instability, vegetative and cognitive disturbances. Medico-social examination demands taking into account the specificity of the disease course depending on the age of its onset.

Gender clinico-social features and the degree of ability to work loss of mentally diseased hospitalized to the psychoneurologic hospital

Abstract

Clinical-social features and degrees of ability to work loss were analyzed by the example of patients staying at the psychoneurologic hospital. Clinical material made 159 patients. Female patients were mostly of retirement age (37.2%), and male patients age was from 21 to 30 (28.5%). Incapable were 3.6% of male patients and 4.7% of female patients. Various forms of schizophrenia prevailed in the structure of hospital pathology; it was equally presented both among males (47.8%) and females (48.8%). Females were significantly rarer registered at the dispensary and rarer violated the outpatient pharmacokinetics regimen.

Abstract

The study of the authors included interviews of about 300 doctors. Interviews have shown that medical expert should possess a certain combination of psychological features necessary to fulfill his duties successfully. Structural logic, which is supposed to predominate in the structure of personality, is the critical quality. Logical-intuitive introvert is an optimal psychological type for expert activity. It is also necessary to create a high quality system of education for experts including basic professional education and expertise training according to the specialization, expert analysis and expert decision-making skills development, individual work in a group of experts, expert decision management, forming a need of self-perfection; psychological training; permanent improvement in basic professional specialization and expertise.

Abstract

Regional features of primary disablement structure of the Nizhniy Novgorod Region adult population according to groups of disablement for the 18-year period were studied. Mean annual values were calculated and dynamics of invalids rate of I, II, II groups in structure of contingent of primarily determined invalids due to certain classes of diseases (general number of follow-up units - 674 137 primarily determined invalids at Medico-Social Expertise Units for the period 1992-2009). Classes of diseases with the most severe incapacitating consequences (circulatory system diseases, malignant neoplasms, psychic and behavioral disorders, tuberculosis) were determined. Knowledge of regional features of the disablement formation process is necessary for optimal management decision-making in forming an efficient, regionally orientated policy in spheres of healthcare and social protection of population.

Abstract

Indices of circulatory diseases incidence and temporary disability and invalidity were studied at the basis of municipal polyclinic ь 195 of the Western Administrative District of Moscow for the period 2004-2007. Among the primarily determined invalids the share of invalids of II group decreased from 73.1% in 2004 to 41.8% in 2007, and the share of invalids of III group increased from 21.6% to 52.7% respectively. The obtained data is evidence of importance of social factor in invalidity determination.

The investigation of cardiac rhythm variability features in patients and invalids with cardiovascular pathology was aimed at studying possibilities of using indices for evaluation of the severity of state and degree of cardiovascular system disturbances and functioning as a whole. It was shown that disturbances of cardiac rhythm neurohumoral regulation were characteristic for the examined patients; significant deviations from norm corresponded to the severity of state increase according to clinical features that confirmed its informative significance for the functional state and functional reserves of the organism characteristic.
