Abstract

The main causes leading to disablement of the infantile population of the Russian Federation and Kursk Region were analyzed in the article. The leading role of the complex of medical and social rehabilitation measures directed to restoration of the lost functions in disabled children was shown. The authors introduced the suggestions on improvement of the organization and fruitful activity of all the interested departments and institutions aimed at integration of children with limited abilities into the society.

Features of medico-social expertise in monocular vision

Abstract

On the basis of the results of comprehensive clinico-ophthalmologic, ophthalmo-ergonomic, psychophysiologic and psychodiagnostic examination the authors developed methodic approach and criteria of medico-social expertise of the patients with monocular vision. Examination of the patients with monocular vision detected abnormalities of ophthalmo-ergonomic, electrophysiologic, psychological data of general capacity for work. The connection of the above abnormalities with age, duration of the disease, group of disablement and the continuance of the adaptation period depending on etiopathogenetic causes of monocular vision was demonstrated.

Rehabilitation of children with high degree of hearing loss and deafness

Abstract

The results of cochlear implantation (CI) in children with auditory abnormalities were presented. During the period from 6 months to 6 years 68 children after CI were followed-up. In 3-4 weeks after the surgery the speech processor was connected up and its repeated settings were performed. Deaf-and-dumb pedagogues and speech therapists worked on development of auditory-speech perception and speech. Auditory-speech skills were examined with use of the simple two-words-phrase test. Limitation of vital functions was evaluated at the Bureau of Medico-social Expertise. In 1 year the data of objective post-op speech recognition were obtained in 18 children. The index of the simple two-words-phrase test made from 60 to 100%, the speech was well developing, and the change of social status was more delayed in time. CI opens wide perspectives for rehabilitation of the deaf children. Integration of such children into the society occurs gradually and depends upon many factors.

Role of rehabilitation in improvement of quality of life physical and psycho-social characteristics in ischemic heart-disease patients due to the change of appreciation of pain

Abstract

Activation of sympathoadrenal system under the conditions of the coronary insufficiency leads to the myocardial ischemia and occurrence of anginous pain. Pain sensations in the heart area in patients with ischemic heart-disease are a permanent source of anxiety and alarm. In connection with that use of psychophysiological self-regulation methods within comprehensive rehabilitation aimed at optimization of emotional state, abatement of painful sensations intensity, improvement of quality of life of the patients suffering from ischemic heart-disease takes on special significance. The conducted treatment has shown that presence of anginous pain in patients with ischemic heart-disease and pronounced psychopathologic symptomatology are accompanied with low level of quality of life. Application of empatho-technique and comprehensive rehabilitation contributed to abatement of pain intensity, decrease of anxiety and alarm level, and improvement of social, role-physical and emotional functioning.

Abstract

The discussion on the importance of development of the individual rehabilitation program for the disabled due to mental diseases undergoing compulsory treatment is presented in the article. The peculiarities of individual rehabilitation program of invalids over 50 were shown. The content of individual program of the above mentioned categories of the disabled in medical, professional and social rehabilitation was revealed. The article emphasizes the importance of individual rehabilitation program for reintegration of invalids into the society.

Abstract

Regional features of the primary disablement age structure of the adult population of Nizhniy Novgorod Region for 10 years (2001-2010) were analyzed. Features of the primary disablement age structure to a certain extent are substantiated by the demographic situation developed in the Region, by regional peculiarities of morbidity and death-rate dynamics. The weight of persons at the age of retirement in the structure of primary disablement in Nizhniy Novgorod Region is one of the highest in the country, and that of young people is one of the lowest. The obtained data made an important information base that should be used taking into account the demographic structure of the population in organization of medical and medico-social assistance in the Region.

Basic tendencies of primary disablement of the adult population of the Nizhniy Novgorod Region due to malignant neoplasms

Abstract

The results of investigation of the dynamics of the level and structure of primary disablement due to malignant neoplasms in the adult population of the Nizhniy Novgorod Region for 10 years (2001-2010) were presented. Unfavourable tendencies of the disablement level increase on the background of growth of primary morbidity, prevalence of oncological diseases and stably high death-rate indices were revealed. The developed situation in the system of medical assistance rendering to oncological patients, level of treatment-and-prophylactics measures on early detection of oncological diseases failed to meet the increasing needs of the population. The results of the conducted study could be useful in planning of organizational and treatment-and-prophylactics measures directed to the increase of availability and quality of medical and medico-social assistance to the oncological patients.

Analysis of the dynamics of primary disablement due to cerebrovascular diseases in the Republic of Mordovia in 2006-2009

Abstract

The article considered the problems of primary disablement indices dynamics according to one of the nosologic forms, i.e. cerebrovascular pathology, in the Republic of Mordovia for 4 years period (2006-2009). The analysis of primary disablement was conducted basing on distribution of the primarily determined invalids due to cerebrovascular diseases according to their age, gender, groups of disablement in comparison with the official data of the Russian Federation and Privolzhsky Federal Okrug. Considering the statistics data we detected a distinct tendency to the decrease of primary disablement indices both in the Republic of Mordovia and in the Russian Federation and Privolzhsky Federal Okrug. Taking into account the severity of the pathology and the high level of the primarily determined invalids of I group the discussion of this problem is topical.

Rheumatoid arthritis: primary disablement in the Irkutsk Region

Abstract

The dynamics of primary disablement in rheumatoid arthritis was studied in the Irkutsk Region for the recent 9 years. Over one half of patients suffering from rheumatoid arthritis who were primarily determined invalids were at the able-bodied age. For the recent 9 years the peak elevation of intensive index of primary disablement in 2005-2006 was connected with social factors and realization of the Federal Law ь12 of 22.08.04. Maximum level of primary disablement in persons of able-bodied age in 2006 (0.57 per 10 thousand people) exceeded twice as much the analogous index of 2003 (0.29). In persons of the age of retirement from 2002 to 2005 the growth of indices made 2.3 times (from 1.06 to 2.48). In the following years the level of primary disablement decreased. In patients suffering from rheumatoid arthritis the severest (I and II) groups of disablement were determined at the age of retirement more often than at the able-bodied age.

Analysis of the disablement in HIV-infection in children in the Perm Region

Abstract

Medico-social characteristic of disablement due to HIV-infection in children in the Region was presented in the article as well as the problems of social support of children suffering from this disease. During 10 years 190 children were examined, at that the status of "disabled child" till 18 years was established. The most vulnerable age was from 0 to 3 years (89.8%); boys prevailed (53%); according to the place of residence these were mostly city-dwellers (82.4%) from incomplete families. Children possessing no limitations of vital functions and not being disabled can benefit from measures of social support according to the legislation of the Russian Federation.

Primary osteoporosis: clinical significance and socio-economic aspects

Abstract

According to WHO data osteoporosis (OP) takes the 5th place among the diseases causing damage to public health. Total risk of fractures leading to clinical cases makes approximately 40% and is equivalent to the risk of cardiovascular diseases development. Fracture of the proximal section of thigh-bone is the most serious complication of osteoporosis, in 86% of cases it is associated with intensification of the risk of following fractures and accompanied by the highest rate of disablement and death. In spite of that in a number of countries including Russia osteoporosis is not entered into the list of priority or socially significant diseases. Only due to preventive measures performed within the framework of the governmental program the incidence of new fractures can be significantly diminished.
