Current issues of medico-social rehabilitation on an outpatient basis: dementia patients

Abstract

Dementia is one of the most common and severe diseases of older age according to its health and social consequences. Almost 95% of patients with cognitive impairment live in families. In addition to cognitive impairment different behavioural and psychiatric symptoms are observed in patients with dementia. The onset of behavioural disorders in the patient family members creates more problems than lack of cognitive functions, and is the most frequent cause of hospitalization, because there is a need in expensive form of medical care. In this regard, rises the significance of adaptation to the conditions of such a person residing in the family and his rehabilitation on an outpatient basis.

The experience of the expert psychological examination of the young child in an institution of medico-social examination

Abstract

The subject of this article is to analyse the main challenges of psychological diagnosis in young children with cognitive and speech disorders undergoing examination in order to determine the category of "disabled child" in the medico-social examination (ITU). The article explains the importance of studying the state of cognitive-play activity in the group being examined. The procedure and priorities of psychological examination are set out, specific procedures designed to diagnose cognitive mental processes and speech of children of the second and third year of life are executed. The expert evaluation criteria of mental dysfunctions in the examined category were proposed. The article presents a case of expert practice, focuses on the ethical and deontological rules when interacting with examined people and their accompanying persons. The difficulties encountered in the course ofprofessional work ofmedical psychology (psychologist) during psychodiagnosis of young children were analysed. We propose an algorithm that helps to optimize, improve performance and comfort of professional interaction with young patients. The necessity of using a professional psychological knowledge and skills for prevention has been justified. There are not enough reliable results ofpsychological testing of the examined category.

Modern aspects of medico-social rehabilitation of patients with mammary gland cancer

Abstract

For the achievement of the assigned aim we carried out a study of quality indicators in 56 patients with a cancer ofa mammary gland to whom the mastectomy concerning was executed. Average age ofpatients was 48 ± 5 years. Term from the moment of surgical treatment was from 1 year to 5 years. To all patients the mastectomy, then chemotherapy and radiation therapy was executed. Level of a depression was defined by Beck’s scale. The special questionnaire including 33 questions was developed for an assessment of indicators of life quality. As a result of the conducted research it is established that patients after the mastectomy executed concerning a mammary gland cancer in the postoperative period complications which influence quality of life of these patients. Thus all complications can conditionally be divided into two groups: 1. The mental connected with depressions development. 2. The complications connected directly with expeditious treatment, chemotherapy and radiation therapy. For the purpose ofreduction ofinfluence ofthese complications by quality of life ofthe operated patients, we offered rehabilitation actions.

Major trends in primary disability due to coronary heart disease in the Russian Federation in dynamics for 10 years (2004-2013)

The paper analyses the level of primary disability due to coronary heart disease based on age and disability groups in Russia in dynamics for 10 years (2004-2013). There is a clear tendency to reduce the level of disability, especially from 2006 to 2013 draws attention to the high proportion ofprimary disability due to coronary heart disease in the structure of diseases of the circulatory system.

Dynamics of primary disability owing to diseases of blood circulation system of the able-bodied age population in Orenburg and Orenburg region

Abstract

The analysis of primary disability of able-bodied population owing to diseases of blood circulation system of the Orenburg region and its administrative center is carried out in period from 2004 to 2013. The comparative characteristic of regional indicators of2004-2012 with the Russian indicators is given. As a result of comparison stable excess of the average Russian level in Orenburg and, especially, in area with its occurrence in 2012 to 15 unsuccessful regions (the 12th ranged place) is revealed. As a whole positive dynamics with a level decrease is noted. Rate of a decrease 2004-2012 in Orenburg corresponds average Russian (27,2%), in area it is less (24,2%) and it is significant less in the loudspeaker 2012-2013. In Orenburg rate of a decrease made 13,5%, in area is 5 times lower (2,9%). Despite active modernizations in health care, including rendering medical care to citizens with cardiovascular pathology, considerable influence on dynamics of primary disability of able-bodied population owing to diseases of system of blood circulation of other medico-social factors, namely, changes in legislative base and in population economic and social situation is noted.

The structure of primary disability due to ischemic disease based on age in the Russian Federation in dynamics for 10 years (2004-2013)

Abstract

The paper analyses the structure of primary disability due to coronary heart disease in dynamics for 10 years with the young, middle and retirement age. The share of young people with disabilities small ranges, reduced to 2009, increased in 2010-2013, the proportion of disabled people of middle age increases; the proportion of disabled people of retirement age is reduced. On average, disabled young adults make up 4.2%, an average of 22.8%, 73% of the pension of the total.

In the article presented the analysis of primary disability due to mental disorders and disorders of behavior of the adult population in the Republic of Mordovia forfive years in the periodfrom 2009 to 2013 has been, taking into account the age, sex, disability groups in comparison with the official data for the Russian Federation and the Volga Federal District. The statistics revealed a clear trend towards reduction ofprimary disability both in the Republic of Mordovia. But given the severity of the pathology and high intensity ofprimary disability indicator the discussion of this issue is relevant.

Drama therapy as a means of rehabilitation for schizophrenic patients

Abstract

The article deals with general psychological and clinical preconditions of the drama therapy use for the combined treatment and rehabilitation of schizophrenic patients. Aims and objectives of drama therapy for the treatment ofpsychotic patients are represented herein. Practical experience of work of a theatre group in a day patient department of a psychiatric hospital is described. Primary stages of drama therapy performance are given. The possibilities of cognitive and negative deficiency correction, increase in motivational component of an activity, correction of emotional and social context of behaviour, decrease in stigmatization of schizophrenic patients are rendered herein. Possible problems in drama therapy performance as a result of scantiness of cognitive and motivational resources of a person are specified. Social orientation of drama therapy and its destigmatizing effects are shown. Primary criteria of drama therapy efficiency are represented, among which subjective satisfaction and improvement in quality of life of schizophrenic patients come first.
