Aspects of rehabilitation and habilitation of disabled persons in modern times

Abstract

In an article there are described new approaches to the development and implementation of the individual program of rehabilitation and habilitation of persons with disabilities at the present stage. Rehabilitation and Habilitation of Persons with Disabilities are directed to the elimination or perhaps a more full compensation of limitation of life activity of disabled persons with the aim of their social adaptation, including them to achieve offinancial independence and integration into society.

Social and hygienic characteristics of the disability and the need in the measures of rehabilitation of the contingent of invalids with lower limbs atherosclerotic arterial disease

There has been presented information about social-hygienic description of 356 invalids with lower limbs atherosclerotic arterial disease with taking into account gender, age-related groups, severity of disability. There were investigated individual programs of rehabilitation of the given contingent ofpatients, dynamics has been followed up for the period from 2010 to 2014 with the analysis of the need for measures of rehabilitation.

Analysis of the main indicators of efficiency of realization of individual programs of rehabilitation of invalids of young age for 2007-2013 in the Ulyanovsk region

Abstract

In an article there is presented the need for the implementation of the rehabilitation of disabled persons of the young age (from 18 up to 44 years) in the Ulyanovsk region in dynamics for seven years (2007-2013) in the main directions: medical, professional and social rehabilitation. There was executed the assessment of indices of the implementation and efficacy of rehabilitation measures. There was revealed a tendency towards to the increase in indices of the efficacy of all rehabilitation measures. The performed analysis allowed to reveal a need for the improvement of the quality of the vocational rehabilitation.

Features of the alteration of poststimulus thresholds of the pain and its subjective characteristics in patients with neuropathic pain in the process of empatho-technique

Abstract

In the work there are presented results of the investigation offeatures of the alteration of thresholds of the post-stimulus pain and its subjective characteristics in patients with phantom pain syndrome and psychogenic pain in the process of empatho-technique. There was studied the role ofpsychophysiological autoregulation in the restoration of sensitivity in patients undergone to amputation of lower extremities. The use of empatho-technique was found to allow not only to significantly reduce the intensity ofphantom, and psychogenic pain, but to fully restore sensitivity to ultrasonic stimuli in the psychogenic pain and significantly eliminate cases of its disturbance in the phantom pain. It is assumed that the reducing influence of empatho-technique on the intensity of the perception ofpain can be realized by means of activation of the antinociceptive system, enhancement of its inhibitory effect on the painful stimulus, as well as decline in the activation of the nociceptive system, with a subsequent decrease in the intensity of sensations ofpain and restoration of sensitivity.

Abstract

The Main Bureau of Medical and Social Expertise in Krasnodar Region, Krasnodar, 350000, Russian Federation This article presents analysis of the dynamics of total disability resulting from traumas of all kinds in persons of all ages in the Russian Federation, the Southern Federal District and its subjects during the 6-year period (2009-2014). We revealed the following trends of disability. First of all, the total disability rate decreased in the Russian Federation, the Southern Federal District and its subjects, especially in 2013-2014. The rate of disability was high in middle-aged persons and low in retirement-age persons. The situation was critical and disability rate was high in the Republic of Kalmykia during the whole observation period. We observed low disability in the Astrakhan Region.

Cancer as the cause of disability in the Novosibirsk region

Abstract

Cancer rate remains to be one out of the highest in Siberian Federal District for many years and it significantly exceeds the mean data across Russian Federation. Besides, it obviously tends to increase. This type of disease has become the first cause of primary disability since 2013 leaving behind cardio-vascular diseases and this ratio is still increasing. Disability caused by cancer is characterized by both the high severity (the share of I-II group disability amounts for 83.9%) and significant percentage of working-age people (37.9%). In spite of high mortality rate caused by cancer, in case of the implementation of the definitive therapy at early stages of the disease rehabilitation indices in these patients remain to be the highest that necessitates for the improvement of the rehabilitation system of cancer patients, including their efficient job placement.

Regional trends in primary disability of retirement age citizens in the Russian Federation

Abstract

In the article there is analyzed the problem of primary disability leave in retirement age citizens in the Russian Federation in the period of2005-2014. In particular, there are presented results of research of main indices ofprimary disability of the given numerous category of population. There are highlighted the prevalence and dynamics of primary disability in the Russian Federation, and features of the level ofprimary disability in individual subjects of the Russian Federation identified in the course of the comparative analysis. There is given the estimation of the gender composition of the cohort of citizens of the retirement age, which initially were defined the disability group in the Russian Federation. There were revealed the subjects of the Russian Federation with the high level of primary disability rate due to social diseases.

Analysis of the cohort of persons rerecognized to be disabled due to the eye and adnexa diseases in the adult population in the Altai region

Abstract

There was performed a study of the cohort of persons re-recognized to be disabled (PRRD) due to the eye and adnexa diseases in the adult population of the Altai region over 2004-2013. In the structure of occasional disability in PRRD the first four ranking places are occupied by such nosological forms as complicated myopia, optic nerve disease, glaucoma and retinal degeneration. Their proportion in the total structure amounted to 71.9%. In the structure ofPRRD according to disability groups with taking into account the gender and nosology,amongpersons having group III and II disability there were dominated male cases with glaucoma (68.7%) and diseases of the optic nerve (64.1%) and female cases with myopia (63.5%). In the structure of PRRD in terms of the age and nosology there were prevailed retirement age patients disabled due to glaucoma (64.9%) and young and middle age patients disabled due to complicated myopia (32.9% and 42.8%, respectively), diseases of the optic nerve (49.8% and 29.9%) and retinal degeneration (34.1% and 26.4%, respectively).

Assessment of efficiency of providing disabled people of the Sverdlovsk region with technical means of rehabilitation and prosthetic and orthopedic products on the basis of the carried-out questioning of disabled people

Abstract

On the basis of the performed selective questioning of disabled people the assessment of the efficiency of providing disabled people with technical means of rehabilitation is given. The vast majority of technical means of rehabilitation (99,1%) is recommended by experts of medico-social expertise within the Federal list of rehabilitation actions, technical means of rehabilitation and the services provided to the disabled person, accepted by the Government of the Russian Federation approved by the order of December 30, 2005 No. 2347-r. Disabled people received the majority of technical means of rehabilitation through Social insurance fund. Other sources of receiving of technical means of rehabilitation are (in the order ofdecreasing ofpopularity): independent acquisition, rental centers, receiving as a gift. In the Sverdlovsk region there is a need ofthe delivery of the regional list of technical means of rehabilitation consisting of the mostdemanded technical means of rehabilitation not included in the Federal list.

The use of oscillometric higly accurate blood pressure monitor for the determination of the ankle-brachial index in examination of patients at suspicion on lower limbs arteries atherosclerosis

Abstract

Atherosclerotic involvement of lower limbs arteries is a common disorder associated with a high risk of cardiovascular morbidity and mortality, requiring an early start of the comprehensive therapy. Typical clinical manifestations of this disease are found only in some patients that encumbers to make timely diagnosis. Determination of ankle-brachial index (ABI) is a reliable approach of the assessment of the state of blood flow in the lower extremities, but the classical technique requires the participation of specially trained personnel and expensive equipment. In the article there is discussed the use of widely available oscillometric blood pressure monitors for the determination of ABI. In a group of 39 patients with high risk of the development of atherosclerosis there was performed a comparison of the results of the determination of ABI with the help of oscillographic blood pressure monitors and ABI evaluated by the method of volumetric sphygmography. There was shown the sufficient accuracy of the determination of ABI with the aid of oscillographic blood pressure monitors and the interrelationship of this index with the risk of detection ofsignificant atherosclerotic involvement of lower limbs arteries according to Doppler ultrasound data. There was demonstrated that this method can be used at the stage of outpatient examination of patients with a high risk for atherosclerotic involvement of lower limbs arteries.

The role of physical rehabilitation in the complex treatment of obesity and overweight correction

In the literature review there are presented current research trends in the management of obesity and overweight correction. There was noted a multifactoriality ofpathogenesis of obesity and the needfor a multidisciplinary approach to the development of comprehensive programs, including surgical, medical and physiotherapeutic techniques. There were determined priority directions ofpathogenetic treatment process, including the correction of diet, optimization of the motor regimen and the use of aerobic physical loads. There was pointed up the necessity for precise dosing of the load for this category ofpatients, there were presented modern methods of the control of its effective and safe intensity.
