The results of effectiveness monitoring of individual rehabilitation programs (IRP) for patients with disabilities in the Nizhny Novgorod region for six years (2005 to 2010). Higher rates of IPR effectiveness were reported in parts of medical and social rehabilitation, the greatest difficulties arise in the implementation of vocational rehabilitation measures in the Nizhny Novgorod region.

Abstract

The study involved 300 people with disabilities of working age who have undergone rehabilitation in the rehabilitation departments of clinics. The content of rehabilitation programs and the institutional structure of the rehabilitation route identified defects of outpatient medical rehabilitation stage: lack of focus on positive social purpose, lack of coverage with necessary rehabilitative effects of disabled, defective organization of the rehabilitation process (irrational sequence of rehabilitation), which reduces the efficiency of rehabilitation effects and fatigue patients. However, it should be noted the positive effect of rehabilitation in all patients.

Abstract

Characteristics of rehabilitation potential and prognosis of rehabilitation of disabled children with cerebral paralysis(CP) within 4-10 years (2002-2010). In dynamics comprehensive evaluation of medical and social rehabilitation of 3,445 children - disabled with cerebral paralysis at the age of 17 years of life, inclusive, has been implemeted. Various aspects of the of the individual rehabilitation potential (PRI) implementation in children with cerebral paralysis, a comparative analysis of the conditions leading to disability before and after rehabilitation have been studied. Identify new approaches to ensure longterm comprehensive rehabilitation system according to the growth and development of the child ’s body. Rehabilitation forecast (RP) in children with cerebral paralysis was evaluated in five age groups with a 5-point scale. Revealed that the formation of IPR disabled children due to cerebral paralysis is determined by the age of the child, health indicators, severity, and clinical patterns of disease, severity of disability and degree of restriction of life, social and economic status of the family, the presence of her motivation for the implementation of the rehabilitation process. Optimization of the PRI and RP contributed significant reduction in the frequency of severe outcomes of perinatal brain lesions in cerebral paralysis in 42% of children and the formation of the less severe forms of disabling disease in 36% of children.

Abstract

The dynamics of the primary disability of the adult population in the Krasnodar region due to cardiovascular disease (CVD) in 1996-2010. Level of primary disability of the adult population of the Krasnodar region due to CVD for the period 1996-2005 increased 1.7 times, but there is a strong tendency to reduce it from 2006. In the structure of the primary disability nosological forms predominate cerebrovascular disease. The structure of disability severity was dominated by the disabled P group, the proportion of disabled W group had a strong tendency to increase, which is a positive prognostic factor.

THE METHOD OF OLECRANON FRACTURE TREATMENT OF ELDERLY PATIENTS WITH THE HELP OF THE INTERLOCKED NAIL

Abstract

The olecranon fracture makes up from 15% till 36% among all the infraarticular fractures. In spite of the numerous methods of conservative and operative treatment, in some cases there can emerge problems connected with the elbow joint function recovery. We have invented the method of olecranon fracturefixation with the help of quadrangular interlocked nail. Since 2011 till the beginning of the year 2012 we have performed 28 operations of such a type. According to statistics, the percentage of early and late epiphenomena has been reduced to 7,1%, and this number is much lower comparing with other treatment methods of sach a fracture category.

Abstract

Indicators of physical (Staten kinetic), somatic, emotional, intellectual, speech and mental development 3445 children with cerebral paralysis (CP) aged 17 or under were studied for the period 2002-2012. Quality of life (QOL) of disabled children with cerebral paralysis according to 10-point scale based on four types of functioning: physical, social, emotional, and role. Life quality comparison according to four compensation levels of motor, mental, intellectual, speech and sensory impairments. There is reduction of all types of operation, particularly severe violations were physical, social and role QOL component, which could indicate, in particular, the welfare and emotional deprivation of disabled children with cerebral paralysis of all ages, especially in the educational institutions in a number of objective and subjective reasons: lack of specialized education and training conditions, a professional staff of insolvency, lack of motivation and psychological support to the family, etc. Life quality of disabled children with cerebral paralysis of all ages can be seen as the final criterion of medical and social effectiveness, psychological and educational rehabilitation.
