Proposals for improvement of the classification and criteria for disability definition

Abstract

The Main Bureau of Medical and Social Expertise in the Altai region, 656049, Barnaul, Russian Federation The relevance of classification discussion and disability criteria establishment is determined by new living conditions in modern society, requiring more concrete legal determination of citizen ’s vital functions limitations, caused by his state of health violation, and methods of their overcoming. An additional incentive to the discussion of this question is the ratification of the Convention on the rights of persons with disabilities by Russian Federation in 2012 and of Ministry labour and social defense of Russian Federation order project «About the classifications and criteria approval, used during realization of medico-social examination of citizens by federal public institutions of medico-social examination» submission, worked out with the use of International classification of functioning, vital functions limitations and health.

Aspects of medical and social rehabilitation of patients with essential hypertension

Abstract

Morbidity, disability and mortality from cardiovascular disease (CVD) have been steadily growing from year to year around the world. Essential hypertension (EH) is the leading cause in the structure of cardiovascular disease. In Russia EH is recorded in 30% of the adult population, and the annual death rate from EH and its complications is more than 1,5 million people. Clinical studies show that there is a close connection between blood pressure and cardiovascular events (acute myocardial infarction, cerebral stroke, cardiac arrhythmias, renal failure). Complications of hypertension are major causes of morbidity with temporary disability, are one of the leading causes of disability and premature death (high mortality). Effective rehabilitation for hypertension requires individualization of therapy, determination of the disease the stage and the degree of organ dysfunction and prediction of potential cardiovascular complications risk. The most effective method of hypertension control is an individualized regular medical therapy in combination with non-drug treatments.

Problems of mental activity expert assessment violations in people with damage to the frontal section of brain

Abstract

The article aims to improve the approaches to the identification and assessment of mental dysfunctions in patients with lesions of the frontal parts of the brain in the bureau of medico-social examination of the mixed profile. The difficulties encountered in medical and social assessment of people with this disorder were analyzed. An algorithm, which promotes the objectification of cognitive and regulatory violations severity and persistence by experimental psychological examination is proposed. A case of expert practice, which caused significant difficulties in determining the body disorders (type, severity and persistence), determining the limits of life in the major categories, formulation of clinical and functional diagnosis and expert determination. The need for uniform criteria of mental dysfunctions assessment has been identified and justified.

Abstract

The technology of the medical and social assessment (MSE) includes patients rehabilitation and expert diagnostics to clarify rehabilitation potential and rehabilitation prognosis, determination of life restrictions severity, the determination of disability group and the subsequent development of individual rehabilitation program (IRP). Unmet needs in almost 100% of people with disabilities in measures of psychological rehabilitation with the need for the vast majority of disabled psychological treatment, psychotherapy and other psychosocial measures, the importance of the experimental psychological examination (EPE) in the procedure of the MSE. Thus, EPE promotes not only objectify clinical and functional status with disabilities, but also more targeted and effective development of the IRP.

According to the study the necessity of modernization of the MSE and rehabilitation has been grounded. Main areas: improvement of patient’s expert - rehabilitation route, expert solutions management, and implementation of individual rehabilitation programs. This requires compliance between Federal requirements of MSE and medical organizations, specialists of medical institutions owned methods of MSE in the structure of the ITU had all the necessary specialists, techniques and equipment for expert diagnosis and rehabilitation, especially for evaluation of disability patients. Five-tier medical education system for physicians in MSE organizations has been introduced.

Permanent disability due to violent causes as the factor influencing the safety of society (On materials of the Arkhangelsk region)

Abstract

The analysis of primary disability dynamic trends due to injuries, poisonings and certain other consequences of external causes in the Arkhangelsk region for the period 2000-2011 and also structure by disability groups was conducted. Frequency of primary disability due to injuries among all population in the Arkhangelsk region is lower by 12,2% of the all-Russian indicator; decrease rate to 2011 is 32,7%. According to the general trends there is an indicator decrease by 43,1 and 49,4% in the adult and separately working groups. Among all causes of primary disability injuries are on the fourth place conceding to cardiovascular diseases, cancer and also diseases of the musculoskeletal system. In structure of disability due to injuries the biggest proportion belongs to the 3rd group. Kinds and causes of primary disability due to injuries, poisonings and certain other consequences of external causes among the population of the town Arkhangelsk for the period 2000-2011 were studied.

The Main Bureau of Medical and Social Expertise in the Altai region, 656043, Barnaul, Russian Federation The subject of this article is to analyze the main challenges of psychological diagnosis of younger students with various kinds of speech disorders undergoing examination in order to determine the category of "child with a disability" in the medico-social examination. The article explains the importance of studying the state of writing in this group. Sets out the procedure of psychological examination, the technique intended to diagnose cognitive mental processes, writing and speaking school children. The evaluation criteria disorders of speech and writing in this category of the examined are suggested. The article presents a case of expert practice, which caused significant difficulties in determining the disorders of the body (type, severity and persistence), determining the limits of life in the major categories, the formulation of clinical and functional diagnosis and expert determination.
